Olustee Battle Festival Arts & Crafts Booth Application
Official Application for Entry

Application is hereby made for a reservation of a space(s) to sell Arts & Crafts February 13th & 14th 2009, in conjunction with the Olustee Battle Festival to be held in Lake City, Florida.  The fee for reserving such space(s) is $110.00 each, plus Florida State Sales Tax, payable with application.  Said fee of $117.70 per space or an end space for $160.50 must be sent to the address shown below before December 15, 2008.  This application is in good faith that you will attend the 2009 festival.  This application is required to be returned no later than December 15, 2008. There is a separate Application Form for Non-Profit Groups. See Below for more information.
