Arts & Crafts Vendor Application

Application is hereby made for a reservation of a space(s) to sell Arts and Crafts February 17th and 18th 2011, in conjunction with the Olustee Battle Festival to be held in Lake City, Florida. The fee for reserving such space(s) is $110.00 each, plus Florida State Sales Tax, payable with application. Said fee of $117.70 per space or an END space for $150.00 plus tax for a total of $160.50 must be sent to the address shown below before December 15, 2010. This application is in good faith that you will attend the 2011 festival. This application is required to be returned no later than December 15, 2010.
