Application is hereby made for a reservation of a space(s) to sell Arts and Crafts February 12th and 13th 2010, in conjunction with the Olustee Battle Festival to be held in Lake City, Florida. The fee for reserving such space(s) is $110.00 each, plus Florida State Sales Tax, payable with application. Said fee of $117.70 per space or an end space for $160.50 must be sent to the address shown below before December 15, 2009. This application is in good faith that you will attend the 2010 festival. This application is required to be returned no later than December 15, 2009. There is a separate Application Form for Non-Profit Groups. See Below for more information.
